While there are many people involved in the process of handing off a patient from the acute care hospital to home care, more and more agencies are using a home care coordinator.
According to the Scope and Standards of Home Health Nursing Practice [2]: A major responsibility of home health nurses is to provide instruction to patients, families and other care providers on acute and chronic disease processes, and to help patients develop other self management skills and abilities. In this role, nurses provide information, demonstrate techniques, and evaluate performance of procedures by patients, families, and other caregivers. Nurses must be able to identify barriers to learning, provide instructions using a variety of methods, and incorporate health beliefs and cultural and religious practices into the process of patient education.
THE HOME VISIT The actual time spent in the patient’s home can be broken into eight steps. Following this process will make the visit organized and focused.
